Durable Power of Attorney - $100

We’ll prepare a ready-to-sign Oklahoma Durable Power of Attorney that lets a trusted person (your “agent”) handle money and property for you. You choose what powers to give, when it starts (now or only if a doctor says you can’t act), and who the backup agent is. Plain English, step-by-step, so banks and title companies know exactly what your agent can do.
You’ll get a customized POA that lists your chosen agent and backup, the powers you want (pay bills, manage accounts, sell or transfer property, deal with insurance/taxes), and your preferred start time (immediate or “springing”). We include a notary block (Oklahoma POAs must be notarized), and one round of minor edits before you sign. This POA covers financial/property decisions—not medical choices (that’s a separate Advance Directive/Medical POA). You can revoke it any time while you’re competent. Notary fees are separate.

Advance Directive / Medical Power of Attorney - $50

We’ll prepare a ready-to-sign Oklahoma Advance Directive (your medical power of attorney + treatment wishes). It names your health-care agent and clearly states what care you want, in plain English.

What You Need to Know

Customized form with your agent and backup, life-support and treatment choices, organ-donation option, and a simple signing checklist—plus one round of minor edits. To finalize, sign with two adult witnesses who aren’t related to you or set to inherit; no notary is required in Oklahoma. Share copies with your agent and doctor.

Basic Last Will & Testament - $150

This service covers basic, uncomplicated wills only—we don’t draft terms for oil & gas/mineral interests, business ownership or succession, complex trusts or tax planning, multi-state property strategies, or special-needs planning. It names your heirs and a personal representative, and can include guardians for minor children and any special distributions

What You Need to Know

You’ll get a customized Oklahoma will that names your heirs, a personal representative (plus backup), specific gifts, guardians for minor children (if needed), and a clear “everything else” clause. We include a self-proving affidavit, a simple signing checklist, and one round of minor edits before you sign. You must be 18+ and of sound mind. To finalize, sign with two adult witnesses at the same time. This service is for straightforward estates (no complex tax planning or trust drafting).

Revocable Trust - $750

I prepare a plain-English Oklahoma revocable living trust for straightforward estates—not complex plans. That means no tax strategies, no oil & gas / mineral interests transfers or business-interest transfers, special-needs trusts, or Medicaid planning. Those sorts of things need the assistance of any attorney to ensure your assets are protected.

What You Get

You’ll receive a customized trust, a pour-over will, a certification of trust, a deed for one Oklahoma home, an assignment of personal property, and a simple funding checklist. I guide you step by step by text and email until everything is signed, notarized, and your deed is recorded with the county. Hometown Legal Forms is not a law firm; if your needs are complex, I’ll tell you up front and recommend speaking with an attorney.

Criminal Record Expungement - $400

What You Need to Know

This service covers standard expungement petitions for eligible misdemeanor or nonviolent felony records under Oklahoma law. Includes: Petition, proposed order, and county-specific step-by-step instructions.
